Notes

The CD is housed in a 20-page, hardcover book that is the same size as a CD jewel case, except a bit thinner. It has a foam rubber piece inside the back cover to hold the CD in place. The book is plastic-wrapped together with a Limited Edition DVD. The front of the CD book is the front of the plastic-wrapped package.

The DVD is housed in a cardboard sleeve that is plastic-wrapped together with the CD book. The back of the DVD sleeve shows the track listing for both the CD and DVD, and is the back of the plastic-wrapped package.
