The indication that it is a promo record on the album cover is that the upper right corner has been clipped (It does in fact differ from a "cutout"). If you look closely, you will notice there is a small triangular notch in the center of the cut. This is as much a Promo Copy indicator as a gold foil stamp, embossed or rubber stamped mark or a sticker.

Includes light blue paper inner sleeve, with folded black and white printed lyric insert with photos (both clipped exactly the same as the cover in the upper right corner)

Note: in runout Side A, the text 'MICHELANGELO WAS A NIGGER' (written two time) is crossed out.
